PHR 4764 - Ambulatory Care II

4 Credit Hours
This elective rotation is designed to further develop students’ knowledge and mastery of skills related to clinical pharmacy practice in an ambulatory care setting. The student will actively participate in direct patient care, including obtaining patient medical and medication histories, performing physical assessments, evaluating drug therapies, developing pharmaceutical care plans, monitoring patients’ therapeutic outcomes, consulting with physicians and non-physician providers, and providing education to patients, caregivers, and health care professionals. This rotation will continue to expand the students’ ability to provide patient care services in collaboration with physicians and other health care providers.

Prerequisite: PHR 4714
