PRX 4004 - Transitions of Care

4 Credit Hours

This elective rotation is designed to introduce students to pharmacy practice as it relates to transitions in patient’s care as they move from one setting of care to another and to provide an opportunity to apply knowledge and skills acquired through didactic education and introductory pharmacy practice experiences. The student will participate in obtaining medication histories, evaluating drug therapies, developing pharmacy care plans, monitoring patients’ therapeutic outcomes, consulting with physicians and non-physician providers, and providing education to patients. This rotation may take place in an inpatient of outpatient setting.
